We finally received a copy of the engineer's letter to repair our foundation after the plumbing was arranged properly.

Subject: Plumbing Penetration thru Foundation
As requested, *********** observed slab penetrations due to plumbing pipe relocation at above referenced address. Slab tendons were exposed but appear to be undamaged except for minor sheathing nicks.
Recommendations:
Tape sheathing nicks with duct or electrical tape. Back fill void to within 6" from top of slab. Install opposed #4 rebar at approximately 6" o.c.e.w. mid-depth of slab with 4" embedment and set with epoxy, then securely tie overlaps and intersections. Replace poly moisture barrier then apply a concrete binding agent to exposed concrete surfaces. Place 3,000-psi concrete into voids then trowel finish.
